Added 3 shipping modules/options! Allow admin to display/hide those option. Also admin can dispaly that option but disable that option for user to know that there are more option but they need to contact the admin to be verify merchnat.

One of the shipping option contain additional information (trucking company, phone #, contact person) will be added to the comments so the admin can contact the provide information and arrange custom shippment.

Got some complaints that the previous Zip package was corrupt.

Tested this one and all seems fine

NOTE: It seems to be still a corrupt ZIP file (at least for some people)

Found a few minor typing errors in the installation document and the findMeTruckingCompany.php file. Repaired and replaced as needed.
Also removed the ADMIN folder and the checkout_shipping.php.

All files for new instalation included.
